PURPOSE-BUILT SENIOR LIVING, ROOTED IN WAUNAKEE
Azura at Waunakee is a new 88-residence senior living community bringing purpose-built Assisted Living and Memory Care to one of Dane County’s fastest-growing communities. Located at 151 Breunig Boulevard within the Heritage Hills master-planned development, the project represents a new joint venture between Three Leaf Partners and Azura, combining thoughtful real estate development with an experienced senior living operator and care provider.
The community will feature 48 Assisted Living residences and 40 Memory Care residences, totaling 116 licensed beds. Designed around Azura’s established household care model, the building will be organized into four distinct households, each with its own residential-style living room, dining area, and kitchen. This smaller-scale approach is designed to create a comfortable, home-like environment while supporting personalized care, meaningful interaction, and a strong sense of community for residents.
Beyond the individual households, Azura at Waunakee will feature a central amenity area, wellness-oriented outdoor spaces and walking paths, raised garden beds, secure outdoor areas for Memory Care residents, and underground parking.

A DESIGN INSPIRED BY THE LAND
The story of Azura at Waunakee begins with the property itself. Located within Heritage Hills, a master-planned community developed by Veridian Homes, the building was thoughtfully designed to reflect the agricultural history of the land and the Breunig family, its prior owners.
Architectural elements throughout the exterior pay homage to the Breunig family’s barns and farmhouse, carrying the character of the original property forward into its next chapter. The result is a modern senior living community that feels connected to its surroundings while honoring the history that came before it.
This attention to place extends throughout the development. From intimate household environments to accessible outdoor spaces and opportunities for connection, every element is designed around the daily experience of residents and their families.
A PARTNERSHIP CENTERED ON CARE
Azura at Waunakee marks the beginning of a new partnership between Three Leaf Partners and Azura and expands Three Leaf’s portfolio of purpose-built senior living communities. Three Leaf Partners and Azura are serving as co-developers, with Three Leaf Construction serving as general contractor.
Together, the teams are creating a community designed to meet the growing need for high-quality senior housing in Waunakee and the greater Madison area while providing residents with an environment centered on dignity, connection, wellness, and purpose.
Construction officially began in September 2026, with completion anticipated in October 2027 and initial resident move-ins expected shortly thereafter.
